What Is Medication Therapy Management (MTM)?
Medication therapy management is a clinical pharmacy service designed to optimize how your medications work for you. A pharmacist reviews your full medication list — every prescription, over-the-counter drug, vitamin, and supplement — and looks for anything that might be causing problems: interactions, duplications, doses that seem off, or medications that may no longer be needed.
MTM was established as a standard of care by the American Pharmacists Association (APhA) and is recognized under Medicare Part D as a covered benefit for qualifying patients. It's one of the most valuable services an independent pharmacy can offer — and one of the most underused, simply because most patients don't know it exists.
The 5 Components of Medication Therapy Management
The APhA framework breaks MTM into five core parts. In plain English, here's what each one means:
- Medication Therapy Review (MTR) — A comprehensive review of all your medications with your pharmacist. This is the main event: a real conversation about what you're taking and how it's working.
- Personal Medication Record (PMR) — A complete, up-to-date list of all your medications in one place. Sounds simple, but most patients don't have one — and it's genuinely useful at every doctor's appointment.
- Medication-Related Action Plan (MAP) — A written summary of anything that needs follow-up. If your pharmacist spots a potential concern or has a recommendation for your doctor, it goes here in plain language you can actually use.
- Intervention and/or Referral — If something needs to be addressed — a possible interaction, an inappropriate dose, a medication that may need to be reconsidered — your pharmacist flags it and, if needed, communicates with your prescriber.
- Documentation and Follow-Up — Everything from the review is documented, and your pharmacist follows up to make sure any action items were addressed. It doesn't end when you walk out the door.
Who Qualifies for MTM Services?
More people qualify than you'd think — and most of them don't know it.
Under Medicare Part D, patients may qualify for MTM services if they:
- Have two or more chronic conditions (such as diabetes, high blood pressure, heart disease, or asthma)
- Are taking multiple covered Part D medications
- Are at risk for high medication costs
If you're a Medicare Part D enrollee and you meet these criteria, your plan is required to offer MTM — typically at no additional cost to you. Many patients receive a letter from their plan inviting them to participate, but you don't have to wait for a letter. You can ask your pharmacist directly.
